Illinois mandate for fertility coverage

Illinois Insurance Code 215 ILCS 5/356m requires group health plans with 25 or more employees to cover IVF and associated fertility treatments, including up to four egg-retrieval cycles. Medically necessary fertility preservation, such as egg freezing for infertility due to medical treatment, is also covered by the mandate.[35]

Self-funded ERISA plans fall outside the state mandate. Illinois patients on self-funded employer plans should check coverage directly. The Illinois Department of Insurance enforces the mandate for fully insured large-group plans.
